Insurance & Payment

Our fees are reasonable and competitive with the market. We participate with most insurance plans. We accept the following plans:
  • Medicare
  • Medicaid (for established patients)
  • Paramount
  • Medical Mutual of Ohio
  • Anthem Blue Cross and Blue Shield
  • Aetna
  • And most other commercial insurance carriers

We are not providers for Medicaid Managed Care plans including:

  • Buckeye Aged, Blind, and Disabled
  • CareSource
  • United Health Care

It is your responsibility to know your medical coverage. If you have questions regarding your coverage, please contact your insurance company. The phone number is usually found on the back side of your card. In order to keep statement and billing costs to a minimum, we ask that you pay for all services at the time rendered. Copayment is expected at the time of service. For your convenience, we accept cash, check, Visa, Mastercard, Discover, and American Express. We cannot accept post-dated checks. Payment plans are available upon request. Please speak to our Billing department for more information. We will submit billing claim forms to your insurance company for processing. Any unpaid portion of your bill is due within 30 days following the final insurance payment. Ultimate financial responsibility rests with the patient. We also prepare claim forms for your motor vehicle accidents, but you will be responsible for submitting the claim to the insurance company or attorney for processing. Please notify our staff prior to the visit if you are being seen for a motor vehicle accident.
